HIV Testing Consent Only applies when ordering an HIV test. The purpose of this form is to document that either my physician or myself has requested that a blood or other bodily fluid test for the Human Immunodeficiency Virus (HIV) be performed. The test(s) are used to detect HIV antibody or DNA. The HIV virus is the cause of Acquired Immune Deficiency Syndrome (AIDS). I understand the test is performed by drawing blood from the arm and processing the resulting blood specimen by a medically accepted procedure, which is extremely reliable. The HIV antibody test will be performed by a licensed laboratory. Pre-Testing Consideration – Many public health organizations recommend that before taking an HIV virus antibody test, a person seek counseling to become informed about the implications of such tests. You may wish to consider counseling, at your expense, prior to being tested.
Disclosure of Test Results – All tests are confidential, except as provided by law. I understand that positive test results must be reported to the appropriate State Health Division. Confirmed HIV antibody test results will be made available; beyond that, only in summary or statistical form in such a way as to preserve my confidentiality. While a positive test result does not necessarily mean that you have AIDS, it does mean that you are at serious risk of developing AIDS or AIDS-related conditions. You may be infected with the HIV virus and infectious to others. You should seek medical follow-up care with your personal health care provider immediately. HIV test results are highly reliable, but not 100% accurate. I have been informed that the tests can produce false positive and false negative results. I have been offered re-testing if it is suspected that this has occurred. If the test gives you a positive result you should consider re-testing in order to confirm the result. If the test gives you a negative result, there is still a small possibility you may be infected with HIV. This is most likely to happen in recently infected persons. It takes at least 4 to 12 weeks for a positive test result to develop after a person is infected, and may take as long as 6 to 12 months. My physician has explained the following to me in language that I understand: the nature of the blood test, its expected benefits, its risks, and alternative tests. By checking the box on the order form, I acknowledge that I have given my consent for the performance of an HIV blood test, and the release of the results as described above. |
